Instructions for Current Clients to Mail Medical to Attorney

To obtain your sealed medical examination, find an authorized USCIS representative here: https://my.uscis.gov/findadoctor

AVOID DR. ERROR – DOWNLOAD BEST PRACTICES FOR DOCTOR


Prepare the following to mail [use full name on file with USCIS & have doctor write name on outside of envelope]:

ORIGINAL SEALED MEDICAL EXAMINATION – IN SEALED ENVELOPE


AN EXTRA PHOTOCOPY OF THE CONTENTS OF THE SEALED ENVELOPE  – REQUEST THIS FROM DOCTOR PRIOR TO SEALING 


THE FOLLOWING ADDITIONAL DOCUMENTS (ONLY IF APPLICABLE TO YOUR CASE):


  • Your original Request for Evidence (mailed to you by USCIS or downloaded in your MyUSCIS account) – IF APPLICABLE

 

  • A PDF copy of the “USCIS curtesy email” requesting Form I-693 (emailed to you by USCIS) – IF APPLICABLE

NOTE:  Most people will not have this “USCIS curtesy email,” and you will not have one if you have a Request for Evidence. It is not the same as the I-693 Deficiency Notice you might have received in the mail. 

 

DO NOT mail using FedEx, UPS or another currier service: 

MAIL USING USPS (UNITED STATES POSTAL SERVICE) PRIORITY MAIL OR EXPRESS MAIL


CHECK BACK ON THE DAY YOU MAIL (address subject to change based on the attorney travel schedule): 

DO NOT MAIL TO OUR DENVER OR GRAND JUNCTION OFFICES !! 

→ This could result in delay and our mail handlers may inadvertently open the sealed envelope.
